An anal fistula is an abnormal tunnel or cavity that connects the inside of the anus to an opening on the skin around the anus.


Most anal fistulas are the result of an infection that has not healed the right way. Anal fistulas can cause unpleasant symptoms and usually do not heal on their own.

Causes of Anal Fistula

Inside the lining of the anus, there are a number of small glands that produce mucus, known as anal glands. At times, these glands get clogged and become infected due to various reasons like injury or abrasion, leading to a pus-filled abscess. When the pus drains away (either spontaneously or surgically), it leaves a small tunnel behind. This drainage tunnel(fistula) remains open and connects the infected anal canal to the outside skin somewhere around the anus.

  • Constant throbbing pain and swelling around the anus.
  • Itchy skin or irritation of the skin around the anus.
  • Bleeding along the stool
  • A foul-smelling blood or pus drainage from anal opening.

Risk factors of anal fistula

The major contributing causes to an anal fistula are anal abscesses. About 50% of these abscesses can develop into a fistula. Other conditions that may cause an anal fistula include:


  • Previous history of anal abscess or fistula
  • Crohn’s disease or other inflammatory bowel disease
  • Infections in the anal glands
  • Trauma to the anal tissues
  • Surgery or radiation for treatment of anal cancer.

Complications of Anal Fistula

Recurrence of an abscess and an anal fistula can happen even after effective treatment. Bowel incontinence (losing control of one’s bowel) is a known complication following fistula surgery, which can significantly impact the quality of life.

Ayurvedic Treatment for Anal Fistula

The Ayurvedic treatment for fistula is rooted in ancient practices, with the Kshar Sutra procedure having been developed around 2,000 years ago. This parasurgical method is used to treat anorectal disorders and is known for being straightforward with minimal complications.

 

The process begins with preparing the Kshar Sutra, where a surgical thread is coated 11 times with Snuhi Latex (Euphorbia neriifolia). It is then coated 7 more times with a mixture of Snuhi latex and Apamarga Kshara (Achyranthes Aspera), and finally, 3 times with Snuhi latex and Haridra churna (turmeric powder). The Kshar Sutra, attached to a flexible probe, is carefully guided through the external opening of the fistula and brought out through the anal canal. The ends of the Kshar Sutra are then tied together and replaced weekly until the tract is cut and healed.
